NYCSHP New Practitioners 2nd CE Program

NYCSHP’s New Practitioner Committee held its second CE program at The Palm West Side in Midtown Manhattan. The venue was at full capacity with over 40 new practitioner members in attendance! The night kicked off with a presentation by Purdue on the topic of Medication Substance Abuse and Diversion. With prescription drug overdoses responsible for more than a million emergency department visits every year, Pharmacists have the unique knowledge, training, and skills to be actively involved in substance abuse prevention and diversion.



After a short break, Dr. Pavel Goriacko took the stage for the main CE Program of the evening. Dr. Goriacko earned his PharmD from Arnold and Marie Schwartz College of Pharmacy and Health Sciences. After graduation, Dr. Goriacko went on to obtain his Master’s in Public Health from Columbia University and is now currently a Clinical Pharmacy Manager at Montefiore Medical Center’s Wakefield Hospital.



Dr. Goriacko’s presentation topic was titled “Population Health: New Challenges and Opportunities for Pharmacists”. As population health management becomes increasingly important in improving overall patient outcomes, Pharmacists must recognize that optimizing patient health involves many aspects of a patient’s life-style and not just medical care. As Dr. Goriacko described, Pharmacists are in a unique position to drive healthcare outcomes, through counseling patients on modifiable patient behaviors, and medication management, for example. Dr. Goriacko spoke about several innovative model designs at Montefiore Medical Center for achieving population health managment goals, including a post-MI Med to Bed program, the Heart Failure Readmission Program and the Care Transition Program.
